Simply click on the Apply Button above, and fill in the required information. Then our customer service will email you the AllCare member card. Just, bring it in for activation and start enjoying the benefits
+ What personal information do you need?
We only need your name, company you work for (if applicable), email address ONLY. Nothing else.
+ Who is qualified to be a member?
Anyone over the age of 18
+ Do I have to pay a membership fee?
It is a completely FREE program! Therefore, no membership fee, or renewal fee
+ Can I share my card?
Yes! But we recommend sharing it only with immediate household members. We encourage everyone to get their own card to ensure nobody is missing out on free sweepstakes and special sales
